Medical Statement
If your child is NOT school-age then we need to have this form signed by a doctor within the past 12 months. To be safe, just take one with you any time your child has an appointment: Medical 

Medication Release
If you would like us to administer ANYTHING for your child, then you need to complete this form first. This is most often used for children’s cold medicine, prescriptions, and sunscreen in the summer time: Medicine
