Medical Statement
If your child is NOT school-age then we need a copy of their latest vaccine record dated within a year of the child’s first day and it will need to be updated annually.
Medication Release
If you would like us to administer medications for your child, then you need to complete this form first. This is most often used for children’s cold medicine, prescriptions, and other over-the-counter medicines.
